Hallux rigidus corresponds to osteoarthritis of the great toe. When cartilaginous lesions are too extensive to preserve the joint, an arthroplasty may be proposed.
The spherical pyrocarbon metatarsophalangeal prosthesis allows preservation of the residual mobility of the metatarsophalangeal joint. It spares the adjacent joints, facilitates the wearing of heeled shoes for women, and the practice of sports requiring this articular mobility.
Unlike other metatarsophalangeal prostheses, the spherical pyrocarbon implant does not seek bony integration, known as osseointegration. Its modulus of elasticity is close to that of cortical bone, and its sphericity allows a harmonious distribution of stresses on the bone.
We use this implant as an alternative to arthrodesis.

Hallux valgus, or bunion, corresponds to an instability of the metatarsophalangeal joint of the great toe. It causes pain and difficulty with footwear fitting. We are the designers of an implant, together with Dr. Pietro Spennacchio, for the surgical correction of hallux valgus.
This implant allows for achieving very significant corrections, limiting associated phalangeal corrections, and reducing the risk of secondary displacement and therefore recurrence, particularly in cases of porotic, fragile bone.
We do not perform surgical correction of hallux valgus for cosmetic purposes or for preventive purposes.
The decision to undergo surgery is yours; it depends on your pain and your disability.
The goal of the procedure is to achieve a stable, painless, and functional forefoot.

Our surgical activity represents approximately 40% revision surgery, meaning patients who have already undergone surgery when we see them in consultation.
The objective of surgical treatment is not the same depending on the patient. There may also be a discrepancy between patients' expectations and what surgical treatment can provide.
In some cases, the functional outcomes achieved do not meet the patients' expectations.
If surgical treatment has resulted in functional improvement and pain relief, a disability may still persist. In that case, if we believe we can provide you with a functional benefit, we will offer you a so-called revision procedure.
If surgical treatment has not yielded a satisfactory result, and you consider yourself still disabled, functional improvement will likely require revision surgery.

We routinely perform our procedures under arthroscopy, that is, using a camera and instrumentation introduced through skin incisions of approximately 5 mm.
The main procedures performed under arthroscopy are:

We perform total ankle replacements in cases of ankle osteoarthritis when the joint cannot be preserved, and in cases of joint deterioration associated with chronic inflammatory diseases.
Prosthetic replacement of the ankle allows preservation of residual mobility, thereby sparing the hindfoot and midfoot joints. Maintaining this mobility can be important for the practice of certain sports such as skiing or hiking.
It is not always possible or advisable to offer an ankle replacement; the alternative is then arthrodesis, which involves fusing the ankle joint. Arthrodesis can be performed arthroscopically, allowing for reduced tissue dissection, decreased postoperative pain, and improved bone fusion.
