What is Brachymetatarsia?
A brachymetatarsia is a congenital anomaly corresponding to a shortening of a metatarsal due to a bone abnormality, or to a premature fusion of the growth plate of the affected metatarsal, blocking the longitudinal growth of the bones.
The involvement of the first metatarsal and the fourth metatarsal are the most frequent, the condition is bilateral in 75% of cases. Brachymetatarsias can cause insecurity because of the aesthetic appearance of the toe length.
More rarely, brachymetatarsias cause pain in the forefoot, especially in neighboring metatarsals. The lack of support on the shorter metatarsal leads to overloading of the lateral rays, which can be responsible for metatarsalgias.
Brachymetatarsias can encourage toe deformities, especially the appearance of an hallux valgus. Finally, the generally elevated position of the toe can make it difficult to wear shoes.
Symptoms of brachymetatarsia
This condition generally results from an incomplete development of the affected bone or a premature closure of its growth plate, preventing its normal growth. Although mainly of congenital origin, brachymetatarsia may also occur as a result of trauma, infection, tumor resection, or surgical intervention.
Patients with brachymetatarsia frequently experience pain in the forefoot, due to an uneven distribution of weight while walking. In fact, since the shortened metatarsal bone does not properly support the weight, the adjacent metatarsals are subjected to overload, resulting in pain called metatarsalgia. This poor weight distribution can also lead to the formation of calluses under the overloaded metatarsal heads, caused by excessive and repeated pressure on these areas.
In addition, the elevated position of the affected toe can cause difficulty wearing certain shoes, due to increased friction on the top of the toe and limited space inside the shoe. These symptoms impact mobility and reduce the patients' quality of life.
Brachymetatarsia Surgery
When pain persists and functional difficulties increase, a surgical intervention may be recommended. The indications are based on chronic pain and problems with shoe fitting. There are two main techniques:
Acute lengthening with bone graft: This technique involves cutting the shortened metatarsal, performing an immediate lengthening, and then inserting a bone graft to consolidate the new length. This approach is suitable for moderate lengthening and uses minimally invasive procedures that allow a fast recovery.
Gradual lengthening with external fixator: For cases requiring a greater lengthening, gradual lengthening is carried out with an external fixator. This device allows gradual distraction of the bone while respecting the natural pace of bone healing. This method provides greater safety for nerve and vascular tissues while preserving bone integrity.

Like any lengthening surgery, surgery for brachymetatarsia carries risks, especially when lengthening more than 40% of the affected metatarsal. The main complications of metatarsal lengthening for brachymetatarsia are:
