Chronic compartment syndrome refers to the increase of pressure in the muscle compartment inside a muscular compartment enclosed by a rigid fascia, causing ischemic muscle distress. In the foot and the leg, the muscle compartments are separated by a non-extensible aponeurosis where the accumulation of edema or a hematoma can lead to compression of the muscles.

Acute compartment syndrome is related to trauma, generally high-energy, and is potentially serious with a poor functional prognosis. Understanding the anatomy of the foot, ankle, and leg is essential to grasp the pathophysiological mechanisms involved in this ischemic syndrome.

There are also compartment syndromes that go unnoticed initially, at the level of the deep and posterior compartments of the leg, potentially responsible for post-traumatic sequelae such as claw toes.

The symptoms and clinical signs of exertional compartment syndrome are characterized by intense and painful discomfort during exercise, localized in the same compartment of the lower limb, accompanied by a sensation of vascular tension and numbness of the foot. The clinical examination may reveal local swelling of the calf, leg, or foot, sometimes causing a slight change in gait and superficial venous insufficiency, as well as muscle cramps and paresthesia, or numbness, during physical activity.

Diagnostic Methods and Additional Examinations

The diagnosis is suspected during the interview and then by a clinical examination. The imaging assessment shows no particular abnormalities. The diagnosis is confirmed by measuring the intramuscular pressure of the suspected compartment using a pressure probe inserted into the muscle compartment. The MRI of the foot or leg makes it possible to assess any potential muscle damage. In cases of distress, the muscle tends to become infiltrated with fat. The MRI thus allows an estimation of fatty muscle infiltration and evaluation of the percentage of fatty infiltration.

Dynamic tests with palpation of the arterial pulse, carried out before and after exertion, help to distinguish this syndrome from other pathologies. In general, in exertional or chronic compartment syndrome, pulses are present and sensory neurological disorders are prominent.

Treatment Options

Patients affected have several therapeutic options, ranging from non-surgical approaches to surgical interventions. As a first-line treatment, conservative management consists of adapting physical activity, introducing periods of rest with elevation of the lower limb, attending rehabilitation sessions including stretching, and, if necessary, using anti-inflammatories and analgesics to relieve pain.

Elastic compression can improve venous return. When this management proves insufficient, a surgical intervention may be considered, usually under local anesthesia, in the form of a minimally invasive fasciotomy that allows pressure in the compartment to be released while preserving the muscle structure. This procedure can be performed using percutaneous surgery.

Chronic compartment syndrome particularly affects athletes who practice activities with repeated impact such as running, mountain biking, circuit motorcycling, or motocross. Runners, football players and dancers can also be exposed to this muscular condition due to the strain placed on their feet and calves.
