Symptoms and Diagnosis

Osteophytes present differently depending on their size, location, and underlying joint involvement. Although these bony outgrowths are often discovered incidentally and remain asymptomatic, they can cause localized pain, felt mainly during movement of the affected joint, as well as stiffness that gradually limits range of motion when the outgrowth hinders normal gliding between joint surfaces.

In some cases, especially when osteophytes develop along the spinal column, their growth can compress nerve roots, leading to neurological symptoms such as tingling, numbness, or muscle weakness in the innervated area.

Early diagnosis of osteophytosis is first based on a careful clinical examination aimed at detecting functional impairment and, when large enough, palpating periarticular bony projections. However, diagnostic confirmation and a precise assessment of the extent of these outgrowths always require an imaging assessment.

Standard x-rays are the first-line examination since they clearly visualize osteophytes and assess their structural impact on the joint. The CT scan provides a detailed three-dimensional analysis, essential for measuring the volume of the outgrowths and evaluating their mechanical impact. Finally, although MRI is less effective for pure bone exploration, it may be prescribed additionally when ligament or tendon involvement is suspected, in order to clarify the diagnosis and guide therapeutic management.

For which conditions is an osteophyte resection performed?

Regarding the foot, the resection of osteophytes is performed in certain cases of hallux rigidus, but also in pathologies such as humpback tarsus where bone outgrowths can create a mechanical conflict. If there is sufficient residual joint cartilage thickness, and if the anatomy of the first metatarsal allows, the procedure that can be proposed consists of resecting the generally dorsal osteophytes, as well as part of the upper portion of the head. This procedure is called debridement with cheilectomy.

Regarding the ankle, the production of osteophytes at the anterior margin of the tibia and talus can be related to ankle osteoarthritis. In this case, resection of the osteophytes can cause decompensation of the ankle osteoarthritis.

In the absence of osteoarthritis, osteophyte resection can be performed alone in cases of mixed anterior ankle impingement.

In cases of chronic ankle ligament injury, the shearing and rotational stresses at the joint are increased, leading to the production of osteophytes. This anterior marginal osteophytosis is resected at the same time as ligament reconstruction.
